746.2409(a)(10) - Safety Requirements for Cribs - Label for Each Infant

Two infants were not using their assigned crib. A caregiver was observed taking out an infant from the crib and placing a different infant in the same crib without prior sanitation.

746.3503(a) - Diaper Changing Equipment - Smooth, Non-absorbent, Easy to Clean Surface

The diaper changing pad in a toddler room was torn. This created the diaper changing mat to become absorbent and hard to sanitize. Note: This was CORRECTED during inspection as the diaper changing pad was replaced with a new diaper changing pad.

1 of 10 children's record reviewed was missing a Statement of the child s health from a health-care professional, and the child's immnuzation record was not current.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Three bottles were not labeled with the infant's first name and initial of last name. Note: The bottles were labeled with the required information during the inspection.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

The director did not have current training in transportation safety. The training certificate reviewed had a date of completion 4/10/2025. Note: This was CORRECTED as the director completed a transportation safety training online during the inspection.

746.1111 - Education Outside of U.S.

A caregiver's education documentation received outside of the United States was not translated into English. Note: This was corrected during inspection as the education documents were translated into English and added to the caregivers record.

746.3407 - Maintenance of Building, Grounds and Equipment

A wooden playhouse was observed with chipping paint and a wooden flower board was observed with some of the chalk paint chipping in the outdoor playground. This posed a safety/health hazard as the areas with chipping paint were accessible to children. Note: The playhouse was removed from the outdoor playground during inspection.

746.5307(b) - Fire Extinguishers Serviced

The fire extinguisher in 2 of 2 transportation vehicles were not currently serviced. The date of service on the fire extinguishers were April 2024 and January 2024.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

Four vents in the operation were observed with dust and debris.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained

Loose fill surfacing (mulch) was not observed to be maintained at the proper depth for a play structure in the outdoor play area. The black plastic underneath the mulch was visible.

746.2431(4) - Infant Daily Report - Infant's Mood

The infant daily report was missing the activities and mood for the day.

746.801(3) - Required Records Maintained and Made Available - Personnel and Training Records

The training records for three staff were not available for review. The most recent health inspection report was not available for review.
