The play structure was less more than six feet did not have required surfacing.

The operation did not have a transportation emergency plan available for review.

Two infants were observed in swaddles. This was corrected when CCR staff informed operation staff that swaddles can not be used without a sleep exception form from a health care provider. The swaddles were removed.

Two children files were missing health statements and have been in care for over 12 months.

The transportation van was not clean and filled with trash, paper, debris, and other clutter on the floor of the van.
