This standard was evaluated in regard to an investigation and a violation was found. There is sufficient evidence to show that a caregiver uses her cell phone for personal use while supervising children.

746.3401(a) - Annual Sanitation Inspection

The operation did not have a current sanitation inspection.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was evaluated in regard to an investigation and a violation was found. A sleeping child was left unsupervised in an empty classroom for approximately 53 minutes while in care at the operation.

746.5401 - Requirement of Gas Leak Inspection

The operation did not have a current gas inspection.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

2 out of 10 children's records reviewed did not have current immunizations.

A child was left completely unattended and unaccounted for within a classroom for up to 30 minutes before being discovered.

A caregiver failed to do a proper attendance check of children in her care while transitioning from one classroom to another. She also failed to identify the issue on her own.

Ten children's records were reviewed and four were found to be missing their immunization records.

Both outdoor playgrounds had exposed nails, uneven wooden logs that presented a tripping hazard, and holes in the ground that presented a potential of injury.

Both playground areas did not have the necessary-loose fill depth.

746.5625(d)(2) - Electronic Child Safety Alarm - Driver or designated employee conduct walk-through and visual check of vehicle

The operation's director did not conduct a walk-through of the bus, which resulted in a child being left on the bus.

746.5605(4) - AP Safety Precautions for Loading and Unloading Children - No Child Unattended

Based on external documentation reviewed during the course of this investigation, the operation's director left a child unattended on the bus.
