747.1309(d) - Primary Caregiver Annual Training - Required Special Topics without specific hours

The primary caregiver was missing topics (d)(5) and and (d)(6).

747.3203 - Sanitation of Building, Grounds and Equipment

The ceiling paint in the daycare has been damaged.

747.301(6) - Notification of Additional Services Offered

Based on the information gathered during the investigation, it was determined that the caregiver failed to notify CCR licensing regarding the new services provided within the required time frames.

747.1501(c)(1) - Responsibilities of Employees and Caregivers -Competency, Good Judgment, Self-control

Based on the investigation obtained, it was determined that the primary caregiver did not use good judgement on two occasions as the caregiver recorded a child?s behavior instead of intervening. In addition, the caregiver chose not to intervene timely during an altercation between children. The primary caregiver demonstrated poor judgment by not effectively communicating with a child.

747.5407(e) - Safety restraint devices for a child must be used when the vehicle is on and during all times when the vehicle is in motion

Based on the information gathered during the investigation, it was determined that children were not wearing seat belts during the transportation service provided by the primary caregiver.

747.2505(1) - Furnishings and Equipment for Pre-Kindergarten Age - Age-appropriate

A three-year-old was seen sleeping on a couch, with a mat under, during naptime. Ths was corrected during the inspection, when the primary caregiver removed the child from the couch.

747.3803(a)(3) - First Aid Kit Incomplete - Antiseptic solutions or wipes

Antiseptic solution was not available at the time of the inspection.

747.3803(b) - First Aid Kit - Supplies Not Expired

The first-aid kit contained some expired supplies. This was addressed during the inspection when the primary caregiver removed the items.
