Cots and mats were not labeled and the operation stated that they labeled the sheets for some children but not all.

746.3503(b) - Diaper Changing Equipment - No Play or Eating Surfaces Used

A toddler was observed being changed standing up by the water bottle table in a common area of the classroom while other children were close by.

746.403(a) - Posting Requirements - Prominent & Public

The required postings were posted behind a door, the operation relocated them to where they could be easily accessible and more visible.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

The operation did not demonstrate good judgement in regard to a child injury. A child sustained an injury and parents were not notified in a timely manner and blood was visible on the nap mat. The child bled for several hours before the parent was notified at pick up.

One caregiver did not have hands-on CPR training.

During opening time, I observed a child who was able to run out the front door and into the parking lot.
