The operation failed to maintain an active franchise tax status, and as a result, the entity?s franchise tax status has been forfeited.

Upon arrival, there were two employees and 13 children present. There was no clear individual left in charge, and when asked, the employee not under any provsions stated that Mr. Jeremy was in charge.

The director noted in interviews that there are times that a particular employee does not abide by all of the conditions placed on their background. They noted that the individual at times takes children outside without other staff members present.

The director admitted in an interview that she was unaware that a staff member that has specific conditions on their background which prevents them from supervising children alone, was not allowed to take their class outside alone without another staff member. She also noted that this does happen at times.

The infant/toddler classroom was observed to have a television displaying Ms. Rachel for a classroom of children mostly under 2 years'-old. 3 of the 4 present were noted as under 2-years-old.
