746.1609 - Maximum Group Size 13 or More Children

A caregiver was observed supervising 14 children ranging from 17 months to 2 years with a Specified Age Group of 18 to 23 months. The ratio for 18 to 23 months is 9:1. The director corrected the deficiency during the inspection by adding a second qualified caregiver in the class to mitigate risk.

746.3417(3) - Children Handwashing After Toileting or Diapering

A child was not guided to wash their hands with soap and running water after a diaper change but instead the child was guided to engage in a table activity. When CCR inspector brought hand washing to their attention, the caregiver guided the child to the hand washing sink to wash the child's hands with soap and running water.

746.3415(4) - Employee Handwashing After Diapering a Child

A caregiver did not wash hands with soap and running water after changing a child's diaper but instead guided the child to enage in a table activity. When CCR inspector brought hand washing to her attention, the caregiver washed their hands with soap and running water.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

This standard was evaluated as part of the investigation and was found to be deficient. When one caregiver saw three children running away from the group while the caregiver was supervising the group with another caregiver, the caregiver did not show good judgement when the caregiver did not run after the children which resulted in the caregiver losing sight of the children.

746.1205(a)(5) - Responsibilities of Caregivers - Intervene to ensure each child's safety

This standard was evaluated as part of the investigation and was found to be deficient. Three children were unsupervised in a classroom when two of the children exited through a fire door to a sidewalk on the outside of the building.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was evaluated for investigation and was found to be deficient. Through review of video and staff interviews, it was determined a child was left unsupervised in a classroom for one minute and 3 seconds.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The Fire Marshall for the City of McKinney has not conducted a fire inspection at the operation since 9/15/2023.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

This standard was evaluated as part of an investigation and was found to be deficient. Through interviews with staff and review of video, it was determined a caregiver grabbed a child's forearm with one hand and pulled the child away from an area when a child was not moving away from the area on their own.
