Based on information obtained during a DFPS investigation it was determined that the standard not compliant. A young child sustained a minor injury to the face and staff could not account and/or know how the child sustained the minor injury.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Two of the six infants were missing current infant feeding schedules.

This standard was evaluated as part of the investigation and was found to be deficient. The operation did not verify that the parents received communication that their child received a less serious injury on the date the injury occurred.

During the inspection, a child in care was observed sleeping directly on the floor.

During the inspection, a sleeping infant was observed with a lanyard in the crib.

A child in care was yelled at by a caregiver.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

An employee without a clear background check was allowed access to children in care. The employee was removed by the operation prior to the inspection.

746.3501(1) - Diaper Changing Steps- Prompt

This standard was evaluated as part of the investigation and was found to be deficient. Based on interviews with parents, it was determined that several children in care were not promptly changed after soiling their diapers or clothes.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver who had an inactive status on their background check was present in a classroom with children. This was corrected at inspection when the caregiver left the operation and the operation submitted a background check for the caregiver.
