746.3407 - Maintenance of Building, Grounds and Equipment

Based on the evidence gathered during the investigation this standard was found deficient. There were 6 out of the 8 licensed rooms for childcare were not maintained and some were used as storage. NOTE: licensed room are no longer use as storage rooms and are maintained.

746.3903(a) - Animals at Center - Documentation of Vaccinations

Based on the evidence gathered during the investigation this standard was found deficient as it was confirmed that 4 cats without vaccination were being fed by the operation staff. NOTE: This was corrected by the time of inspection as the cats were removed from the operation.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

746.901 - Required Personnel Records

Three staff files were reviewed. Two out of three files reviewed were not available for review at the time of inspection.

746.4305 - Fenced Outdoor Activity Space

The gate is broken from one end of the fence which does not lock and accessible to the children at the time of inspection. The gate to the outdoor fence has a whole in the bottom accesssible to the children.

746.1301(a)(4)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric CPR

A caregiver counted in child/caregiver ratios does not have a current CPR training. Expired on 10/2025.

746.2705(1) - School-age Furnishings and Equipment - Age-appropriate Seating, Tables

Observed a school age classroom being used as a storage room at the time of inspection.
