746.1601 - Child/ Caregiver Ratio - 13 or More Children

One infant room was over ratio by one child. There were 6 children with one caregiver. This was corrected at inspection when one child was moved to another classroom which placed all classrooms in ratio.

746.5605(4) - AP Safety Precautions for Loading and Unloading Children - No Child Unattended

During a DFPS investigation, it was determined that a child in care was left unattended in a vehicle for two hours.

746.5625(c) - Electronic Child Safety Alarm - used at all times when transporting a child in care

During a DFPS Investigation, caregivers conducting transportation admitted to not using the electronic child safety alarm as required. In addition, owner stated they did not require the caregivers to use the electronic child safety alarm. This resulted in a child in care being left unattended in a transportation vehicle.

746.1205(a)(1) - Responsibilities of Caregivers - Know children responsible for

During a DFPS investigation, it was determined that a caregiver counted in ratio admitted to not knowing she was responsible for a particular child in care. This caregiver conducted transportation and did not know this child was in the transportation vehicle. This resulted in the child being unaccounted for and left alone and unsupervised in a transportation vehicle for two hours.

746.5625(d)(2) - Electronic Child Safety Alarm - Driver or designated employee conduct walk-through and visual check of vehicle

During a DFPS investigation, after reviewing video footage at the operation, a caregiver conducting transportation was seen exiting the transportation vehicle and not conducting a physical walk-through. This resulted in a child in care being left alone and unattended in a transportation vehicle for two hours.

746.5617(a)(1) - Required Equipment in Vehicles - List of Children

During a DFPS Investigation, it was determined that a caregiver conducting transportation admitted to not having a list of the children being transported. This resulted in a child in care being left unattended in a vehicle for a period of two hours.

746.5625(b) - Electronic Child Safety Alarm - installed and maintained according to manufacturer's instructions

During a DFPS investigation, it was determined that the electronic child safety alarm in vehicles used for transportation were not functioning according to the manufacturer?s instructions.

746.5401 - Requirement of Gas Leak Inspection

The operation has not been inspected for gas leaks in the last two years. The last gas leak test was completed on 8/19/2023.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Two infants did not have updated written feeding instructions.
