746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

Two of the eight staff files reviewed were missing a notarized affidavit for review.

746.1301(a)(5)(A) - Training Requirements for Employees, Caregivers, and Directors - 24 Annual training

When reviewing the eight staff members' 24 hours of yearly training, it was found that three staff members were short on their required training hours. One staff member was short 2 hours in emergency preparedness. A second staff member was short 6 hours in emergency preparedness, administering medication, preventing the spread of germs, as well as culture and diversity training. The third staff member did not complete 12 hours of required training in topics such as SIDS (Sudden Infant Death Syndrome), teacher-child interaction, the emergency preparedness plan, administering medication, and cultural diversity.

746.503 - Provide Parents With Copy of Policies

Two of six children's records reviewed did not have documentation that parent's were provided the operational policies.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

A teacher demonstrated poor judgement by allowing children access to a sprig of berries in the classroom, that was later found to be poisonous.

746.1203(4) - Provide care consistent with child's habits, interests, strengths, special needs, including any special supervision needs

A child with a diagnosed food allergy did not have an epinephrine pen available at the operation as required by the child's Food Allergy and Anaphylaxis Emergency Care Plan.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Two caregivers in the primary classroom gave the incorrect number of children in their care. Note: This was corrected at inspection when caregivers confirmed the number of children in the classroom
