During the walk around of the operation I observed 2 active ant hills.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

One child's file did not have the original exemption affidavit. One child's file did not have an exemption affidavit; however, there was a written exemption request.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained

The loose fill in the use zones of both climbing structures needs to be maintained as there is grass and weeds growing through the loose fill. Also the loose fill needs to be maintained to ensure the 6 inches of depth.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

The feeding instructions for one infant have not been updated within the last 30 days.

746.3407 - Maintenance of Building, Grounds and Equipment

On the playground the following was observed: Several Broken little tyke cars were observed on the playground. A tunnel toy was observed with the tunnel part caved in and broken. The slide on the large play structure was cracked and broken at the exit of the slide. The small play structure had rusted platforms. The tick tack toe toy had several cracked and broken parts. The yellow truck stationary equipment had paint peeling.

Two classrooms had paint chipping on the walls.

746.605(6) - Required Admission Information - Emergency Contact

One child's file had the mother as the emergency contact instead of an alternate adult. Note: The director spoke to the mother and was able to update the contact person.

746.3503(a) - Diaper Changing Equipment - Smooth, Non-absorbent, Easy to Clean Surface

In the infants, wobblers and 2's classroom the changing table mat had rips in them. Photos were taken.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

A caregiver was observed using their cell phone while responsible for supervising children in care.
