This standard was reviewed for this investigation. Based on the information received during the investigation, an infant younger than twelve months of age was sleeping in a crib on their stomach.

A teacher in the toddler classroom in direct contact with children. doesn't have the requied background check.

Owner was present at the operation today after being notified that she isn't allowed to be present. The owner had to be instructed to leave the facility.

It was observed during today's follow up that the children were playing on an outdoor play area while chickens were walking around the play area. This was corrected at inspection by staff rounding up the chickens and moving the children to another play area.

A teacher in the toddler 1 classroom was observed across the hall at the door of another classroom leaving her class unattended.

The operation isn't complying with the safety plan put in place during a current abuse and neglect investigation stating that they would not bring liquids into the classroom. Three classrooms had beverages inside the class.

Based on information received during the investigation conducted by DFPS, it was determined the incident report wasn't completed by the caregiver present at the time of the incident. The report included inaccurate information about how the incident occurred. The report also was not signed by the child's parent.
