The driver did not conduct a walkthrough or visually check the vehicle. The alarm system, which is designed to ensure that the caregiver completes this check, was not functioning at the time. As a result, a child was left on the vehicle.

The caregiver failed to verify that all children were safely unloaded from the vehicle, resulting in a child being left on the vehicle.
