Based on interviews conducted and external documentation obtained during the investigation it is determined that a caregiver forcely grabbed an infant and forecly placed him on the floor during an incident while in care.

Based on information obtained during a DFPS investigation, it was determined that a caregiver inappropriately placed napping equipment, requiring the caregiver to lean/step over a child to get to the bowl of hot soup in the microwave.

Based on information obtained during a DFPS investigation, there were findings of neglectful supervision due to a child sustaining third-degree burns as a result of a caregiver unintentionally spilling hot soup on a child.

Based on information obtained during a DFPS investigation, a caregiver did not use good judgment when they decided to warm up soup in a microwave in the classroom not intended for caregiver use.
