This was evaluated as part of a DFPS investigation and found deficient. Through interview it was found that there were 2 incidents where concerns were raised about unexplained bruises on children in care. There was no report made to Childcare Regulation regarding incidents of this nature.

This was evaluated as part of a DFPS Investigation and the standard was found deficient. In video reviewed it was found that a caregiver thumped a child on the cheek multiple times.

This was evaluated as part of a DFPS investigation and found deficient. It was reported after video review that a caregiver forced a child to drink milk after the child had dumped food on the ground.

This was evaluated as part of a DFPS investigation and found deficient. It was found through the investigation that a caregiver demonstrated poor judgement and self control when they touched a child inappropriately.

This was evaluated as part of a DFPS investigation and found deficient. As a result of the DFPS investigation there was sufficient evidence to find the caregiver had sexually abused a child in care.
