Based on the evidence gathered during the investigation, the operation did not notify CCR within the required two days about a child receiving medical treatment to remove a bead from inside a child's nose.

Based on the evidence gathered during the investigation, it was observed in a video two children walking and running around with training cups in their mouths and hands.

Based on the evidence gathered during the investigation, it was observed in the videos caregivers changing diapers but did not wash any of the children's hands after the diaper change.

Based on the evidence gathered during the investigation, it was observed in a video two children exited the classroom unsupervised. A different caregiver subsequently escorted the children back inside.
