746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

During the visit, there were four bottles that were not labeled in the infant classroom.

746.801(3) - Required Records Maintained and Made Available - Personnel and Training Records

During the monitoring visit, three caregivers did not have all their training records available. The trainings that were not available were guidance discipline, age-appropriate curriculum, teacher-child interaction, shaken baby syndrome, prevention of SIDS, child maltreatment, emergency preparedness, preventing the spread of diseases, administering medication, food allergies and reactions, and building and physical safety.

746.5625(b) - Electronic Child Safety Alarm - installed and maintained according to manufacturer's instructions

During the monitoring visit, the transportation vehicle safety alarm was not working. The vehicle is used to transport children to and from school.

746.5617(a)(2) - Required Equipment in Vehicles - Emergency Medical Information

Three children did not have the emergency authorization filled out on the application. The children are transported to and from school.

746.1301(a)(3)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric First Aid with Rescue Breathing

During the monitoring visit, one caregiver did not have a current First Aid certificate available for review.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

During the monitoring visit, one caregiver did not have a current CPR certificate available for review.

746.1311(a) - Director Annual Training 30 Hours Required

During the monitoring visit, the director did not have any annual training hours available for review.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

During the follow-up visit, there was one caregiver that did not have a background check. The background check was inactive. The director submitted the background check during the visit. The caregiver was not able to leave the operation due to the number of caregivers present to supervise the children in care at the operation.

746.501(a)(18) - Written Operational Policies - Promotion of Physical Activity

The parent handbook did not include the promotion of play section that covers the times and the importance of physical play, as well as the dress code.

746.625 - Immunization Records for School-age or Pre-kindergarten

Three school-age children's admission forms did not complete the school section. Verify shot records are at the local school.

The director and two caregivers did not have a current CPR/first aid certificate available for review during the monitoring visit.

746.501(a)(30) - Written Operational Policies - Support inclusive services

The parent handbook did not include the supporting inclusive services for young children.

The director and a caregiver did not have an updated transportation training certificate available for review during the monitoring visit.

During the monitoring visit, I was not able to view any director training hours from the annual year.

746.2207(c)(4) - Screen Time Activities - Not Used During Eating or Rest Times

During the monitoring visit, the television was on during snack time. The children were watching a cartoon. The caregiver in charge turned the television off upon request.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

During the monitoring visit, one child did not have a food allergy emergency plan available for review during the monitoring visit.

746.603(a)(4) - Children's Records - Immunizations

One child did not have a shot record available for review during the monitoring visit.
