Based on information obtained it was determined that the operation did not make adequate adjustments to supervision in the classroom to prevent children from becoming too rambunctious and causing injuries.

746.5401 - Requirement of Gas Leak Inspection

At the time of the inspection the operation had not had a gas inspection completed in the required time frame.

A caregiver sprayed a child with soap and water while cleaning the tables.

This was evaluated as part of a DFPS investigation and found deficient. Based on information provided the operation did not create an incident report for an incident where a child was found in a closet inside of a classroom.

746.305(a)(6) - Report Situation Placing Children at Risk

This was evaluated as part of a DFPS investigation. Based on information provided it was found that a child was in a dark closet space inside the classroom while the caregiver was changing diapers. The operation director and staff were aware of this happening and made no reports.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This was evaluated as part of a DFPS investigation and found deficient. Based on information provided a caregiver could not maintain visual awareness or ability to intervene when a child was shut in a dark closet inside the classroom. The room is used to store nap mats and other class items.

At follow up it was discussed that caregivers are able to use a child lock on the closet doors in the classrooms and it was discussed with caregivers that these should be closed so children do not enter.

This was evaluated as part of a DFPS investigation and found deficient. Based on information provided a caregiver was assigned to a classroom where multiple employees stated the caregiver had difficulty managing.

At the time of the inspection 1 child with a food allergy was not included on the posting of food allergies in the child's classroom.

3 of the 10 children's records reviewed did not have a health statement from the child's health care provider.

At the time of the inspection 1 child with a food allergy did not have a food allergy action plan on file signed by the health care physician.

At the time of the inspection 4 of the 10 employee records required the 2912 form and it was not signed or present in the employee's file.
