746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

During the course of a DFPS investigation, a teacher admitted using the word bully to a child due to the child?s behavior towards other children. Also, the teacher would yell at the children due to her being frustrated. This employee is no longer employed.

746.2905(1) - Children Not Required to Sleep - May Not Force Child to Sleep

During the course of a DFPS investigation, a teacher admitted to placing her legs over a child and guiding the child's head down on the mat to prevent the child from getting up during naptime. This employee is no longer employed.

746.901(5) - Required Personnel Records - Required Affidavit (Form 2912)

All employees did not have a pre-employment affidavit available for review. This was corrected when the director had all those in the building read and sign a copy.

746.3417(4) - Children Handwashing After Outdoor Activities

The 12-2-year-old class was observed coming in from playing outside and not washing hands when entering the room.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

Transportation training from the director and the employee who drives the van was not available for review.

The last health inspection was conducted on February 28, 2024.

Two rooms were observed without outlet covers. This was corrected when the covers were placed in the outlets.

The 2-year-old class was observed coming in from playing outside and not washing hands when entering the room.

All fire extinguishers observed were last serviced in November 2023. The owner called the company to set up an appointment which will be on May 19th.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

The teacher in the toddler room did not give the correct number of children she was responsible for at the time of the inspection. She stated she was had 10 and she had 11 children. This was corrected when the roster was checked by the owner.

746.4751(b) - Maintenance Procedures for Active Play - Monthly Maintenance Checklist

The monthly maintenance checklist was last done the week of April 14th.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The last fire inspection was conducted on December 4, 2023.

746.3407 - Maintenance of Building, Grounds and Equipment

A plastic container in the 2-year-old room that holds toys was damaged, leaving rough edges that could pose a risk to children. This was corrected when the container was replaced. Several nap mats in the toddler room had the padding exposed.

746.5625(c) - Electronic Child Safety Alarm - used at all times when transporting a child in care

A van used to transport children the child safety alarm on the vehicle was not working during the inspection.
