746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

During an inspection I observed a baby sleeping in a bassinet type bed on top of a baby lounger pillow, wearing a bib and having a pacifier string/clip attached to the baby's clothes. The pillow was removed along with the bib and picifier clip. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

746.603(a)(3) - Children's Records - Health Statement

There were two children files who were missing health statements.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

Observed an infant that did not have written feeding instructions.

746.603(a)(4) - Children's Records - Immunizations

Observed 6 children files and 3 were missing immunization record or needed an updated immunization record.

746.3407 - Maintenance of Building, Grounds and Equipment

Observed piles of equipment on the playground that could fall on the children. There were chairs and other equipment stacked up.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

Observed a slide on the playground that did not have any loose fill. This was corrected when the slide was taken off the playground.

746.4505 - Storage for Individual Belongings

The operation did not have individual places to put their belongings. The operation did purchase hooks to hang the childrens items.

746.2409(a)(1) - Safety Requirements for Cribs - Snug Fitting, Firm, Flat Mattress Designed for Use in Crib

Observed 2 pack and plays with loose fitting sheets.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Observed a caregiver with children in care go to the kitchen to make breakfast, leaving the children without a caregiver in the classroom.

746.605(15) - Required Admission Information-Child's allergies and Completed Food Allergy Plan

The operation did not have the food allergy form for a child in care who had food allergies. The operation did have a list of food allergies for the child but did not have a signed allergy form from a health care professional letting the operation know what steps to take if the child was to have an allergic reaction. This has been corrected as the child no longer attends the operation.

746.3301(i) - Nutrition and Food Service - Must not serve identified foods to allergic child

The operation provided food to a child who was allergic to the food that was provided. The child no longer attends the operation.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Observeed two caregivers in a classroom with 7 children, one caregiver was asleep on the floor and the other was on their phone. This was corrected when the caregiver woke up and the other got off of their phone.

746.901(2) - Required Personnel Records- Meets Requirements

Observed two caregivers files without proof of education.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Observed two caregivers on one playground with 2 children on their phones. The other 6 children were on a different playground. This was corrected when the phones were put away and the caregivers put all children in one playground.

746.901(5) - Required Personnel Records - Required Affidavit (Form 2912)

Observed one caregiver that did not have a pre employment affidavit.

746.1309(a) - Documented Annual Training Area - 24 Hours Required

Observed training records, two caregivers were missing the following trainings, Caregiver 1- Child growth and Development, Discipline and Guidance, Age appropriate corriculumm, Teacher child interaction, Emergency preparedness, Communicable, Administering medication, Food allergies, physical building premisis safety, hazardous. Caregiver 2 Teacher interactions, emergency preparedness, communiccable disease, adminstering medication, food allergies, physical and building premises safety and hazardous materials.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

No one at the operation had a current first aid or CPR. Their training had expired in 9/2025.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

Observed a 5 month old infant sleeping in a restrictive device (bouncy seat). This was corrected when the caregiver placed the infant in the pack and play.

746.3701(10) - Safety - Televisions Anchored to Prevent Tip Over

Observed a television sitting on a shelf, I spoke to the caregiver about having the television mounted to the wall. The television was removed from the classroom.

746.2419(8) - Requirements for Feeding Infants - Other Use of Handwashing Sink

Observed a caregvier rinsing cups in the handwashing sink. I spoke to the director regarding not rinsing the cups in the same sink that she washes hands in.
