746.3701 - Safety - Areas Free From Hazards

Based on information gathered during the investigation, a mat on the playground had a large tear it in, causing a child to trip and fall.

746.305(a)(6) - Report Situation Placing Children at Risk

The operation failed to report two different incidents in which children in care were discovered to have been left unsupervised/unattended by operation staff to CCR as required.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During the course of the investigation, it was determined that a two-year-old child was left unsupervised on the playground for approximately two minutes. when the child's class returned indoors.

746.2401(7) - Basic Requirements for Infants - Never Unsupervised

Based upon evidence collected during the course of the investigation, it was determined that an infant was left alone sleeping in a crib, unsupervised, for approximately 10 minutes when the rest of the class went outside on the playground.

746.2405(2) - Infant Care Area Furnishings and Equipment - Crib for Each Non-walking Infant Younger Than 12 Months of Age

Three non-walking infants under the age of twelve months old were observed sleeping on a crib mattress on the floor.
