During the course of the investigation, it was determined that a bathroom door was broken and fell on a child's hand which resulted in a child being injured.

During the course of an investigation, it was determined that a caregiver failed to use good judgement when a child received a critical injury that required medical attention and the caregiver decided to not call 911 which delayed the child receiving immediate medical attention. A caregiver also did not use good judgement when the parent was notified of the injury. The caregiver did not express the extent of the injury of the child to the parent.

During the course of an investigation, it was determined that the operation failed to call 911 when a child received a critical injury.

The infant 1 room classroom was out of ratio by one child. There were 5 infants, ages 0m-11m with one care caregiver. The specified age of that group is 4 infants, 0m-11m, to one caregiver.

Three out of 10 caregivers lacked the required 24 hours of annual training. One caregiver lacked 7 hours, the second caregiver lacked 23 hours, and the third caregiver lacked 20.5 hours of training.

During the course of an investigation, it was determined that a caregiver was threatening to lock kids in a closet.

During the course of the investigation, it was determined that a caregiver was spanking children, pinching children, and hitting kids in the back of the head.

During the course of an investigation, it was determined that a caregiver left another caregiver with 17-18 children, and their ages were 18m-2 years old.

During the course of an investigation, it was determined that a caregiver was holding children by their feet for discipline practices and was throwing rocks at children.
