One caregiver did not have CPR training that adhered to AHA guidelines.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

Two out of three staff who attended to incident of child in care did not have a current CPR/First aid or no CPR/First Aid available for review. One staff hired on 2/2025 and the other on 4/2025.

The cook staff was present while the background check status was pending.

Based on the information gathered during the investigation, it has been determined that children were left unattended, and caregiver was not at a physical proximity which did not allow visual and/or auditory awareness of the children in care.

Based on the information gathered during the investigation, it has been determined a caregiver did not demonstrate good judgment when she placed a wrong diaper on a child.

Ten out of eleven classrooms did not have the cots labeled.
