746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Feeding instructions for six infants was observed to not be updated for the required every thirty days.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on preponderance of evidence, there sufficient evidence to support the allegation of a child being left unsupervised in the hallway of the operation.

746.305(a)(6) - Report Situation Placing Children at Risk

Based on the information obtained, the operation attempted to notify licensing of an incident that a child left was left unsupervised in the operation. The operation made the self-report four days later after the operation attempted to contact licensing of the incident.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation has not complete their annual fire inspection report since 08/31/2023.

746.3809 - Medication No Longer Used

Observations were conduct inside the medical cabinet on where the medication and prescribe medicine is kept and store at. Two prescribe medications were found to be expired and two children are longer enrolled inside the childcare center.
