During the course of a DFPS investigation, it was determined that two caregivers failed to demonstrate good judgement in the execution of their duties. The caregivers failed to do a count of children and did not conduct a name to face count, which resulted in a child being unsupervised and left outside for approximately 16 minutes.

During the course of a DFPS investigation, it was determined that a child was left alone on the playground. The child exited the playground and was found banging on the door. The child was unsupervised for approximately 16 minutes.
