During the walkthrough, it was found that the operation was out of ratio with 7 children in care and one caregiver. There was a 5 month infant, two children who are 2 year olds, two children who are 21 months, and two children who are 3 year old.

The director did not have30 clock hours of annual training hours for the training year of July 18, 2024 to July 18,2025.

It was found that the caregiver/director did not have an applicable CPR training with the use of a manikin. The caregiver/director had a NationalCPRFoundation certificate that offers online training for CPR and no manikin.

Two of the four people on the employee list are no longer asscoiated with the operation and have not been employeed for over a month.

A full personnel file was not made available for review.

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

A climbing play structure and seewsaw did not have proper surfacing underneath as they were set over grass.

One infants feeding instructions had not been reviewed and/or updated since March 2026.

746.4313(2) - Additional Requirements For Outdoor Activity Space Not Connected To Center - Written Plan for Supervision

A playground safety plan for the detached play area was not made available for review to show the safe route children take to get to and from the play area that is across a parking lot.

It was found that the number of employees that left in 2025 was not reported by 1/15/26.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The Fire Marshal had not conducted an inspection withing the past year.
