One classroom was over ratio by one child during the inspection walk through. This was corrected at inspection when one child was moved to another classroom.

Two children's enrollment forms indicated they had a food allergy, however there was not a food allergy plan on file. One child had the allergy medication at the operation but the other child did not.

During a DFPS investigation, it was found that children were not being appropriately supervised when children were jumping on cots which resulted in one child injuring their wrist.

During a DFPS investigation, it was found that a caregiver did not use good judgement when they did not report a suspected injury to the director or parents in a timely manner when a child was showing signs of a hurt arm which resulted in the child having nursemaid's elbow.

During a DFPS investigation, it was found that the operation did not notify Child Care Regulation within the required two days when they were made aware a child required medical treatment when they injured their elbow after jumping on cots.

A changing table was observed to have a basket with clean sheets and bibs on top of it during inspection. Some of the clean sheets were directly touching the surface of the changing table. This was corrected at inspection when the items were removed from the top of the changing table.
