Two out of five children were missing parent signatures for Epi pens.

746.3809 - Medication No Longer Used

Two types of medication was found to be expired. This was corrected when the medication was put away to give back to the parent .

746.3703(d) - No Smoking or Use of Tobacco Including E-Cigarettes or Vaporizers

Based on the information gathered, a caregiver entered the facility with a vape pen in her shirt pocket. The pen fell out of her shirt resulting in a toddler picking it up and putting it in the mouth. It was retrieved quickly from the caregiver. NOTE: The caregiver with the vape pen was terminated.

746.2419(1) - Requirements for Feeding Infants-Hold through 6-months

A teacher was observed feeding two infants without holding them. The two infants were observed on the floor mat being fed. This was corrected when the teacher held the infant to feed.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on the video and interviews conducted, two children were left in the classroom unsupervised for approximately 5 minutes when the caregiver walked out of the classroom with another child.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The last fire inspection was conducted on 4-11-2024 which is past due.

746.3401(a) - Annual Sanitation Inspection

The last health inspection was conducted on 4-17-2024 which makes it past due.

Based on the video and interviews conducted, a staff did not conduct a name to face before walking outside in front of the children allowing the classroom door to shut. This resulted in a child being left in the classroom unsupervised for approximately 4 minutes.

746.3407 - Maintenance of Building, Grounds and Equipment

The plastic containers in two rooms that hold toys were damaged, leaving rough edges that could pose a risk to children. This was corrected when the containers were removed. A few rooms had torn diaper changing mats were the foam was exposed. This was corrected when the mats were removed.

Two forms of medication were expired. This was corrected when the director disposed of the medication.

Based on the video and interviews conducted, two staff did not conduct a name to face while lining the children up to go inside from the playground and when entering the room to ensure all children were accounted for. The staff did not do a sweep of the playground. This resulted in a child being left outside on the playground unsupervised for approximately 9 minutes until another class came outside and found the child.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

A caregiver was heard yelling at the children while she was changing a child's diaper.
