746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

One teacher stated that they had 12 children in care but had 11 children.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

During an investigation it was found that a caregiver used their personal water bottle to squirt water at a child's face when the child squirted the caregiver with water.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

During an investigation it was found that caregivers are on their phones while they are supposed to be supervising the children in care.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

During an investigation it was found that a caregiver is grabbing children in an aggressive manner for discipline.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

During an investigation it was found that caregivers are yelling at the children in care.

746.3407 - Maintenance of Building, Grounds and Equipment

It was observed two bathrooms had toilet paper clugged in the toiled, and also there was toilet paper in the ground, and missing toilet paper as well. It was corrected during inspection.

746.3317 - Food Service and Preparation Requirement - Safe and Sanitary

During an investigation it was found that the food was cold when being served to the children and had to be taken back to the kitchen to be rewarmed.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

During an investigation, there was evidence to support the allegation that caregivers are yelling at the kids in care.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During an investigation it was found that a caregiver went to sleep while caring for children. It was also found that a child was left in a classroom by themselves for approximately 1-2 minutes. There was no evidence to support the allegation that a child was carrying around adult size scissors.

746.1003(1) - Director Responsibilities - Operate in Compliance

It was found that the operation is not in compliance with the minimum standards regarding ratio's when the director is present in the mornings.

746.201(5) - Reporting Suspected Abuse, Neglect, Exploitation-May not delegate responsibility to report

During investigation it was found there were caregivers aware of multiple allegations of suspected abuse, which was not reported by the operation. This was corrected when we spoke to the operation's staff about how and when to make reports.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

During an investigation it was found that multiple classrooms have been out of ratio more than one time. It was found that the classrooms are mostly out of ratio on Wednesday, Thursday and every other Friday mornings.

During an investigation it was found that a caregiver grabbed a child by their arm for discipline.

During any investigation there was evidence to support the allegation that a caregiver would yell at the children for discipline

746.1201(5) - Responsibilities of Employees and Caregivers-Report Suspected Child Abuse, Neglect or Exploitation

The operation did not report a suspected abuse within the required 48 hours.

746.5613 - One Seat Belt Per Person

During an investigation, it was found that 2 children had to share a seat on the bus during after school pick up on at least one occasion. The operation has changed their transportation procedure in response to this incident.

746.613(a) - Required Immunizations

One child was missing one immunization.

746.611(a)(1) - Required Health Statement - Written, dated within past year, indicates child is physically able to participate in operation's child-care program

One child did not have a physician statement indicating the child was well enough to attend the child care operation.

746.3401(a) - Annual Sanitation Inspection

The operation did not have a current sanitation inspection.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

A fire inspection had not been completed for the operation since 2023. A sprinkler system test was completed, but after calling the city, it was determined that the operation had not been inspected since 2023.
