There were areas inside that posed health and safety risks to the children. Peeling paint was observed and dust was observed in one classroom.

746.5625(d)(2) - Electronic Child Safety Alarm - Driver or designated employee conduct walk-through and visual check of vehicle

During the inspection, it was found that the operation staff had asked the children to turn off the alarm upon arrival at the operation.

Peeling paint was observed in a room where the children were napping, dust was observed on the ceilings near the vents, and there were tripping and entanglement hazards on the playground. Boards on the 2 year old playground was damaged and a nail was exposed, and there is surfacing tarp that poses a tripping hazard.

A caregiver did not use good judgment and self-control when choosing to discipline a child in care. A caregiver picked a child up and dropped the child to the ground.

The operation's staff did not have access to employee pre-service or training files for child care regulations to review.

746.401(2) - Posting Requirements - Letter or form from the most recent Licensing Inspection or Investigation

A copy of the most recent inspection was not posted and visible for parents.

During the investigation and review of video, it was determined that the caregiver did not interact positively with children in care.

Employee records were not available during the investigation inspection.

During the investigation, it was found that the caregiver lacked self-control and yelled at the children in care.

During the investigation, it was found that the caregiver yelled at the children in care.

During the investigation, it was found that the director did not make a CCR report by calling the intake line.

During the investigation, it was found that the position of the caregivers was not within physical proximity or full view.

746.307(b)(4) - Parental Communication - Situation that Placed a Child at Risk

During a DFPS investigation, it was found a serious incident occurred with a child in care on June 13, 2025, and the operation failed to inform the parent of the child.

746.2805(4) - Prohibited Punishments - Putting Anything in Mouth

During a DFPS investigation, it was found a caregiver, who admitted having been frustrated, placed saran wrap around a child?s face and head.

746.305(a)(6) - Report Situation Placing Children at Risk

During the DFPS investigation, sufficient evidence was found that several operation staff became aware of a serious and inappropriate incident involving a child in care and failed to report it to CCR. The incident occurred, and the operation became aware of it on 06/13/25.

746.2805(1) - Prohibited Punishments - Corporal Punishment

During the DFPS investigation video footage was obtained and showed a caregiver aggressively grab the arm of a child in care, aggressively push another child's arm, and put their face into a child's face in an intimidating manner.
