746.2805 - Prohibited Punishments - No Harsh, Cruel or Unusual

During the investigation inspection it was found that a caregiver used inappropriate methods of discipline on children. It was found that a child was placed against the wall for time out and the caregiver was seen tapping another child on the arm.

746.611(a)(3) - Health Statement parent sign - Dated within past yr, name/address health care professional, states child can participate. Follow by signed statement

2 of 8 child files evaluated had admission forms indicating the child had been examined in the past year by a healthcare professional and that a health statement would be provided within 12 months of admission but there was not documentation in the files that the health statement had been provided within the 12 months.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

One caregiver had first aid/cpr that expired at the end of November 2024. Their renewal has not been completed yet,

746.3407 - Maintenance of Building, Grounds and Equipment

A blue dome on the playground was observed to be peeling in several spots, Note: Photos were taken.

746.2805(9) - Prohibited Punishments - Withholding Active Play

Information gathered in the investigation did not substantiate that a child had to sit out from playing outside for twenty-five minutes for talking. Information gathered in the investigation substantiated that children have had to sit out from playing outside for short periods of time for behavior that occurred inside as a consequence for behavior.
