746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

Cleaning supplies were stored under a sink cabinet without a safety latch in the infant room. The cleaning supplies observed were two cans of glass cleaner, two cans of Lysol disinfectant spray, a spray bottle of OdoBan disinfectant and three scrubbing brushes. This posed a safety hazard as the cleaning supplies were accessible to children. Note: This was addressed with the caregivers and all cleaning supplies were removed from under the sink cabinet and stored in an area inaccessible to children.

746.307(c) - Parental Notification of Less Serious Injuries

Based on the information obtained during the investigation, it was determined that the operation did not notify parents of less serious injuries when the parent picks the child up from the child-care center.

746.5307(a) - Inspect Fire Extinguishers Monthly

The fire extinguishers were last inspected in May of 2025, per the documentation on the Emergency Preparedness Form. The fire extinguishers were inspected during the inspection, in order to bring the deficiency current.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Two infants in care did not have updated feeding instructions on file.

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

There were 3 slides that did not have unitary surfacing or loose fill around the fall zones.

746.4003(b) - First Aid Kit - Supplies Not Expired

The alcohol prep pads that were in the main First Aid backpack in the hallway were expired as of July 31st, 2023. This was corrected during the inspection when the alcohol prep pads were removed and replaced.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

The 30 day Infant Feeding updates that are signed by the parents could not be located. The director developed new documents for the parents to update and sign.
