The operation did not have a current health inspection. The date of the last health inspection was 2/18/2025.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

It was determined through the course of the investigation that 2 children on two separate occasions were left unsupervised. One child was left unsupervised in a classroom and the other was left unsupervised on the playground.

746.305(a)(6) - Report Situation Placing Children at Risk

It was determined through the course of the investigation that operation did not notify CCR when 2 children on two separate occasions were left unsupervised. One child was left unsupervised in a classroom and the other was left unsupervised on the playground.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

It was determined through an investigation that portions of the walls in the recreation room were in disrepair exposing sheetrock.

746.3407 - Maintenance of Building, Grounds and Equipment

It was determined through an investigation that the operation has not been being maintained in a sanitary manner. Floors, carpets, windows restrooms and other areas throughout the facility are visibly dirty and contain debris.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

During the review of video surveillance, it was determined that a caregiver transitioned from outside to inside unknowingly leaving a child in their class behind on the playground with another group of children.

746.1205(a)(5) - Responsibilities of Caregivers - Intervene to ensure each child's safety

During the review of video surveillance, a caregiver was observed leaving a group of 9 toddlers on the playground when she stepped inside the doorway of the building making her unable to intervene and no longer in close proximity

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

During the review of video surveillance, a caregiver was observed using a cell phone for approximately 2.5 minutes while supervising 9 children on the playground.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

A caregiver in the 2 year old room did not have an active background check. This was corrected at inspection when the caregiver left the premises of the daycare.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

11 infants in the 12-17 month old room were observed to be unbuckled in the toddler tables during meal time. This was corrected at inspection when all the infants were buckled in bringing the operation into compliance.

746.801(11) - Required Records Maintained and Made Available - Fire Inspection Report, Including Written Approval for Above/Below Ground Level Care

The operation did not have a current fire inspection on file for review during the inspection.

746.2207(a) - Screen Time Activities - Not Used for Under Two

The infant room had a screen playing coco melon during lunch time that was visible to the 4 infants in the room sitting at the toddler table. This was corrected at inspection whenthe screen was turned around and no longer visible to the children in care.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The fire drills were last completed on 6/18/2025. The operation completed a drill during the inspection bringing the operation into compliance.

It was determined through the course of the investigation that a Pre-k child was left unsupervised.

A caregiver in the pre-k room was using their cell phone for personal use during nap time.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

There are portions of the wall in the main hallway, large indoor play area, and upstairs hallway that paint is peeling and baseboards are peeled back exposing the sheet rock. Furthermore, the licensed restroom downstairs is missing ceiling tiles.

746.3701(10) - Safety - Televisions Anchored to Prevent Tip Over

The television in the school age classroom is not anchored.

A fire drill has not been conducted since 12/31/2024.
