746.3701(10) - Safety - Televisions Anchored to Prevent Tip Over

During the unannounced inspection, a television monitor was observed on topp of a shelf in the preschool classroom and was not mounted to secure it and prevent it from tipping over. The Director removed the television from the classroom, so this was corrected during the inspection.

746.3701(1) - Safety - Electrical Outlets Covered

During an unannounced inspection, the electrical outlet in the preschool classroom was observed to have the cover plate removed, exposing the area around and behind the outlet. This outlet is on the lower part of the wall and is easily accessible to the children.

746.2207(a) - Screen Time Activities - Not Used for Under Two

During an unannounced inspection, a television was observed in a mixed-age classroom that includes children under the age of 2 years, which was confirmed to be used at least once per week on Fridays. Any use of screen time is prohibited with children under 2 years of age. The Director removed the television from the classroom, so this was corrected during the inspection.

746.2805 - Prohibited Punishments - No Harsh, Cruel or Unusual

During the course of the investigation, it was determined that a caregiver used prohibited punishment when she hit a child with a toy to demonstrate how it hurt and threw a ball at the child.

A copy of the incident report was obtained during an unannounced inspection. The report form was incomplete and was not signed by the operation or the parent.

One employee who is on provisional status was observed to be alone with a child during the inspection. This provisional status requirements were reviewed with the Director.

10 employee files were reviewed during the inspection. All 10 were lacking required information and are incomplete.

746.801(10) - Required Records Maintained and Made Available - Safety Documentation for Emergency Drills, Fire Extinguishers, & Smoke Detectors

The record of emergency drills was reviewed during the inspection and was not complete. The operation has conducted one fire drill in March 2025.

The Director records were reviewed and the required transporation training has not been completed.
