A water hose which presented a tripping hazard was observed on the PreK outdoor space. This was corrected when the water hose was removed.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

Observed a dime size hole in the toddler classroom's wall.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Based on the information obtained, it was determined that a caregiver was not demonstrating good judgement when playing with the children in care. This was corrected when the caregiver was removed from the operation.

746.1403(a)(1)(A) - Training requirements for substitutes, volunteers, and contractors-Orientation

Two contractors did not have orientation to the child-care center.

746.1401(e) - Substitutes, volunteers, and contractors who do not meet caregiver qualifications must never be left alone with children

Two contractors were left alone with children in their careand counted in ratio who do not meet caregiver qualifications. A caregiver from the operation joined the group to supervise the children during the inspection so that the children were not alone with the contractors.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

The loose-fill did not have the required depth when measured during inspection.

746.4803 - Measurement of Use Zone for Stationary Equipment

Three pieces of stationary outdoor equipment did not have enough use zone space when measured during inspection.

Two children's immunization records did not have the signature (including a rubber stamp or electronic signature) of the health-care professional who administered the vaccine.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

One caregiver did not know how many children she was responsible for during inspection. This was corrected when the number of children present was brought to the caregiver's attention and confirmed during inspection with the caregiver.

746.1315(b)(1) - CPR Training and Recertification Adhere to Guidelines-skill based

One caregiver had CPR training that was completed online.

746.4813(a) - Measurement of Use Zone for Rotating and Rocking Equipment (Sitting)

Three pieces of outdoor rocking and rotating equipment did not have enough use zone space when measured during inspection.
