The pre-k/schoolers playground area loose fill was not maintained properly, in which dirt was exposed.

746.5607(b) - Child Passenger Safety Seat System - Not expired or damaged in accident

The safety seats observed in two transportation vehicles were expired. Safety seats observed were manufactured ranged from between 2011-2015. One safety seat observed expired, 03/03/2018.

746.2409(a)(2) - Safety Requirements for Cribs - Snug Fitting Sheets

Two cribs in the infant classroom did not have snug fitting sheets during the walkthrough. One infant was observed napping in one out of the two cribs observed with loose fitting sheets. Sheets on the infant mattress were replaced during the inspection.

746.3701 - Safety - Areas Free From Hazards

Several bags of mulch were observed on the pre-K playground area near the slide and underneath the play structure, posing a risk for a tripping hazard to children. While Licensing staff was present, the mulch bags were removed from children access.

Of the 8 personnel records evaluated, one has not completed 24 clock hours of pre-service training

The diaper changing surface in the infant classroom has multiple tears' therefore it is no longer non-absorbent.

The operational policies lacked the following information: 1. Procedures for parental notification 2. Information regarding field trips 3. Parents Rights 4. Procedures for parents to review and discuss concerns with the director 5. Procedures for parents to participate with the center's activities 6. Information on how parents may access Minimum Standards and the correct HHSC website. 7. Staff vaccination policy 8. Procedures to support inclusive services for children with special care needs.

Activity plans were not posted in two classrooms. This was corrected at inspection.

Cleaning supplies were accessible to children in the Pre-K classroom. This was corrected at inspection.
