Information obtained during a DFPS investigation shows that the operation failed to assign a caregiver to an age group that matched their ability to safely and effectively meet the care of non-verbal children.

Information obtained during a DFPS investigation shows that the operation failed to notify parents of a less serious injury.

746.305(a)(2) - AP Report Child Injury Requiring Medical Treatment by a Health-Care Professional or Hospitalization

Information obtained throughout the investigation shows that the operation failed to report the incident to Child Care Regulation within 48 hours. The operation reported the incident via email, but did report by either speaking with someone or reporting on the abuse and neglect website within 48 hours.

746.613(a) - Required Immunizations

14 out of 17 children immunizations were not up to date.

746.3417(3) - Children Handwashing After Toileting or Diapering

A caregiver failed to wash an infant's hands after changing their diaper. This was corrected when the caregiver washed the infant's hands.

746.3301(b) - Nutrition and Food Service - Follow Meal Patterns

During snack time children were only served food from one food group which did not meet the USDA nutritional guidelines.

Two caregivers did not know how many children they were responsible for.

Four out of ten children files were missing the address for the emergency contact.

Two classrooms had cleaning supplies that were accessible to children. This was corrected when the caregivers moved the cleaning supplies out of reach of children.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Information obtained throughout the investigation shows that the operation failed to demonstrate good judgement by continuously allowing children to enter a hallway closet that self locks and having to get a key to unlock the closet and let the children out.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver without a background check eligibility letter had direct access to children in care. This was corrected when the caregiver did not return to the operation from their lunch break.

746.2207(a) - Screen Time Activities - Not Used for Under Two

Information obtained throughout the investigation shows that caregivers allowed children under the age of 2 years to participate in screen time activities.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

Operation did not have a current fire inspection.

746.3401(a) - Annual Sanitation Inspection

Operation did not have a current sanitation report. This was corrected during my inspection when I received receipt of payment for the inspection to be completed.
