Based on the interviews with the staff, parent, child witness, the victim, and the video, the operation was found not to be in compliance with the standard evaluated. A young child found a small round object in the gym and placed the small round object in their nose. The young child received treatment from a health care professional to retrieve the small object out of the young child's nostril.

746.2909(1) - Arrangement of Napping Equipment - Not Blocking Entrances or Exits

A child in a primary classroom was asleep on their cot placed in front of the back door. This was corrected at inspection when the caregiver moved the child to a different area in the room.

746.2911 - Adequate Lighting During Naptime

A primary classroom had a curtain hanging over the window causing the room not to have adequeate light in the classroom during naptime.This was corrected at inspection when the caregiver removed the curtain from the window.

746.1613(a) - Reduction of child/caregiver naptime ratio by 50% allowed, If 13 or more children in care, children are 18 mo. or older and other standards met

A classroom reduced the ratio by 50% with a 12 month old present in the group ranging from 12 to 24 months. The ratio was 9 to 1. Ratios during nap time can only be reduced by 50% for children 18 months and older in a group. The deficiency was corrected during the inspection when a second teacher was placed in the classroom to supervise the children.

746.4907(c) - Installation and Maintenance of Loose-fill Surfacing - Posts Marked to Proper Depth

The equipment support post were not marked to ensure the surfacing is maintained at the appropropriate depth.
