This standard was evaluated during the investigation and was found to be deficient. Drinking water was not made available for a child during snack time and during active play.

746.2805(2) - Prohibited Punishments - Associated with Food, Naps, or Toilet Training

This standard was evaluated during the investigation and was found to be deficient. A child was denied water by a caregiver due to the child having a toilet training accident.

746.603(a)(4) - Children's Records - Immunizations

One out of ten child records reviewed did not have an immunization record on file.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

During the inspection, cleaning supplies were accessible to children under five in three different classrooms or their attached bathrooms.

The Director has not prevented the operation from suffering discipline-based deficiencies in 2025 on multiple occasions.

During the investigation, it was found the operation had been placing two-year-old children into a chair with a belt attached to prevent the children from engaging in disruptive behaviors.

This standard was evaluated during a DFPS investigation and was found to be deficient. A caregiver grabbed and pulled a child by the wrist while disciplining the child.

746.2805(10) - Prohibited Punishments - Using Inappropriate Long Period of Silence or Inactivity Including Requiring Child to Remain in a Restrictive Device

This standard was evaluated as part of a DFPS investigation and was found to be deficient. A caregiver used an inappropriate form of discipline when a three-year-old child was required to remain standing and inactive for twenty minutes, as a form of discipline.

746.3501(1) - Diaper Changing Steps- Prompt

This standard was evaluated during the investigation and was found to be deficient. A caregiver did not change a child's diaper promptly.

746.1309(e) - Documented Annual Training- Required Content

Ten out of ten staff training records did not have training in the storing, handling, and disposing of hazardous materials.

746.2909(1) - Arrangement of Napping Equipment - Not Blocking Entrances or Exits

During the inspection a child was napping on a cot during nap time which was blocking an emergency exit. This was corrected during the inspection when staff removed the cot from in front in front of the exit.
