746.2207(a) - Screen Time Activities - Not Used for Under Two

During the investigation the operation was found to be deficient with this Minimum Standard. Interviews conducted during the investigation confirmed children under the age of two years of age were being shown videos of Ms. Rachel and Jack Hartmann which constitutes screen time prohibited for that group age.

746.305(a)(2) - AP Report Child Injury Requiring Medical Treatment by a Health-Care Professional or Hospitalization

Caregivers were aware of that a child had recieved medicatal attention to remove a rock from the child's nose and did not report the injury to licensing. I spoke with the staff about making reports and how to make reports. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

The operation did not provide the parent with a copy of the incident report.

The operation learned of an incident that occurred rendering it unsafe and did not report to regulations or the abuse hotline.

A caregiver did not conduct a name to face identification to ensure all children in care were accounted for, prior to re-entering the building

Screen time activities was accessible to children under the age of two years old. This was corrected when the caregiver turned the television off.

Three files were observed and did not have the medical authorization sections filled out.

During a DFPS investigation it was found that staff failed to review the groceries ordered online upon their delivery to ensure the items were correct to what was ordered, this resulted in peanut butter crackers being served to a child that is known to have a peanut allergy, causing an allergic reaction.

746.1109(a)(1) - Caregiver Qualification Age Exemption with HS Diploma/GED- Not in Charge of Group

During a DFPS investigation it was found a group of children were left in the care of a staff member that is a minor.

746.605(15) - Required Admission Information-Child's allergies and Completed Food Allergy Plan

During a DFPS Investigation, it was found that the operation had not received an emergency allergy plan from the parent for a child with stated allergies.

746.305(a)(3) - AP Report a Child In Care Showing Signs and Symptoms of Illness Requiring Hospitalization

During a DFPS Investigation, it was found that the operation did not notify Childcare Regulations that a child in care was hospitalized after having an allergic reaction.

746.2805(6) - Prohibited Punishments - Using Harsh, Abusive or Profane Language

A caregiver was seen and heard yelling at a child on the playground.

746.2405(4) - Infant Care Area Furnishings and Equipment - Hand-washing Sink

There was a sink in the infant room that did not have water in it. Sink was refilled during inspection. There was no sink in room 13 for anyone to wash their hands. Director has a protable sink that she will put into classroom.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2024 data from January 16, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
