746.801(5) - Required Records Maintained and Made Available - Employees' Attendance

Attendance records for the director were not available for review at inspection.

746.3415(13) - Employee Handwashing After Removing Gloves

A caregiver was observed doing multiple diaper changes while wearing gloves but did not wash hands after removing gloves in between children having their diaper changed.

746.605(15) - Required Admission Information-Child's allergies and Completed Food Allergy Plan

The food allergy plan in file for a child that has a diagnosed food allergy plan was not complete as it was missing the symptoms if the child eats the food allergen and the treatement for this as well as the parent signature.

746.305(a)(6) - Report Situation Placing Children at Risk

The operation failed to report a situation that placed a child at risk within the required 48 hours of the incident occurring. A child was left unsupervised in the playground area without any staff supervision and the primary caregivers were not aware of the child's ongoing activities and did not have visual or auditory awareness.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for when the child was left behind outside in the playground for less than 3 minutes. Additionally, the child was discovered by another individual followed by the director and then the staff were made aware that the child had been left behind and was not with the group of children they were responsible for.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

Two staff did not have the required orientation in file within 7 days of employment. Two staff did not have the required pre-service in file for review when requested. One staff file was lacking both orientation and pre-service training in file when avaiable.

746.1309(a) - Documented Annual Training Area - 24 Hours Required

A caregiver employed did not have all required 24 clock hours in the annual training year and was lacking 13 hours.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

Three caregivers did not have the required notarized affidavit in file, form 2985.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for when the child was left behind in a classroom during a transition time. Additionally, the child was discovered by the parent and staff when the staff was made aware that the child had been left behind and was not with the group of children they were responsible for.

746.1613(a)(6) - Naptime Child/Caregiver Ratio - Caregivers Physically Present

Based on the evidence obtained, a caregiver has not been physically present in all areas when children are laid to nap.

The monthly playground checklists that were last available for review were dated for July 2024.

Additionally, the child was discovered by the parent and staff were made aware that the child was missing.

746.2805(1) - Prohibited Punishments - Corporal Punishment

Based on the evidence obtained during the investigation, a caregiver popped a belt against the floor close to where children were. This scared the children and appears to have been done in a threatening manner as a form of discipline to redirect the children.
