Electrical outlets in one classroom where observed not covered and accesisble to children. The same classroom 18- 23 months had a surge protector was acdesible to children. In the schoolers (4 and up) classroom the television cords are exposed and pluged into a surge protector left on a table within reach of children.

An infant 12 months old was observed in a crib with a loose fitting sheet. Other cribs where observed with loose fitting sheets.

Bleach spray bottles where observed in 2 toddler classrroms and after school classroom within reach of children. (toddler classroom by the handwashing sinks, afterschoolers room in the window connected to the kitchen.)

Infant classroom was observed to have one caregiver with 5 infants a specified age of 0-11 months.

Classrooms were observed to be out of ratio. According to sign in and out logs classrooms are out of ratio routinely througout the day.

Operation staff did not show good judgement by incorrectly informing a parent of an incident.
