746.501(a)(20) - Written Operational Policies - Parent Rights

Parent rights were not fully included in the updated parent handbook.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

A new hire non-exempt caregiver who requires pre-service training has not obtained the first 8 hours of pre-service training before being counted in child/caregiver ratio.

746.501(a)(26) - Written Operational Policies - Preventing and Responding to Abuse and Neglect

All the required information for abuse and neglect was not including in the parent handbook such as warning signs, trainings, prevention methods, and community engagement/resources.

746.1301(a)(3)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric First Aid with Rescue Breathing

Two new hires for the toddler room did not have first aid training prior to having unsupervised access to children while in care.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on DFPS documentation obtained, proper supervision was not maintained when 3 children were able get out the toddler playground gate and into the parking lot unsupervised.

746.2805(1) - Prohibited Punishments - Corporal Punishment

It was self-reported and found that a 1 year old was inappropriately disciplined when a caregiver hit the child's hand out of punishment.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

Children were sitting in seats and were not strapped in. The seats were missing the safety straps. This was corrected at inspection.

746.3317(7) - Food Service and Preparation Requirement - No Foods That Present Choking Risk

Children under the age of 4 were served whole wieners, whole grapes and whole cherries which is a choking hazard.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

One caregiver did not know the ages of the children they were caring for and did not have this information showing each child's age.
