A caregiver did not have proof of 24 hours of annual training on file.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

An infant was observed sleeping in a restrictive device. The caregiver stated that the infant had been asleep for approximately 30 minutes. The caregiver proceeded to remove the infant from the restrictive device once notified of the deficiency.

746.5537(a)(3) - Electronic or Battery-Operated Carbon Monoxide Detector-Document Test Date, Battery Installation Date, and Staff Name Testing and Installation

Testing of the downstairs and upstairs carbon monoxide detectors were not documented. The detectors were tested during inspection and were working.

The vehicle used for transportation on the day of inspection did not have a working thermometer.

746.5401 - Requirement of Gas Leak Inspection

The 2 year gas leak inspection is past due. The last inspection was on 3/6/2023.

One caregiver who transports children did not have current transportation training. This was corrected with the caregiver completing the training during the inspection.

One employee had access to children in care prior to the operation receiving a background check result of eligible, eligible with conditions, or provisionally eligible. The employee was excluded from being present at the time of inspection.

The annual health inspection was past due.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

It was determined that there was sufficient evidence that a caregiver grabbed and pulled a child by the arm and shirt to redirect the child when not following the caregiver's directions.
