746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

During the course of a DFPS investigation it was found that an infant was asleep in a restrictive device.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

At the time of the incident the caregiver was unaware of the location of the infant. The caregiver believed the infant was still near the toy bin in the middle of the room.

746.3701 - Safety - Areas Free From Hazards

An infant was able to access a staff only area that had an exposed metal pipe. The infant was able to crawl in-between the infant area and the staff only area multiple times.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

A caregiver used poor judgement by pouring hot water into a sink with exposed metal pipes, resulting in an infant receiving a second degree burn when the infant touched the metal pipe.

746.3607(2) - Critical Injury - Give First Aid or CPR

The operation failed to timely provide first aid to a second-degree burn. Additionally, the operation used unidentified ointment on the infants burn.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

The infant care sheets were reviewed and five were found to be signed in July.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

A caregiver was unaware of how any children were in her care. This was corrected when caregiver used their name to face (Pro care) to verify the children in their care.
