746.603(a)(4) - Children's Records - Immunizations

At the time of inspection one child did not have a copy of their immunization record or health statement on file for review.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was tasked over for this investigation and found to be deficient. The caregiver in charge of the class at the time of the injury was observed not properly supervising the class or outdoors on the playground and did not notice the child's fall that caused the injury. The caregiver did not observe a change in the child's appearance or actions while present in the classroom.

746.1315(b)(1) - CPR Training and Recertification Adhere to Guidelines-skill based

While reviewing personnel records, it was determined that a caregiver had completed a CPR course online that did not include a hands on component to the training.

746.3701(1) - Safety - Electrical Outlets Covered

During the walk through an uncovered electrical outlet in the 18 to 24 month classroom was observed in plain sight. The director corrected the issue at the inspection by placing a safety cover on the electrical outlet.

746.901(9) - Required Personnel Records - Photo Identification

While reviewing personnel records, it was determined that 4 of the employee folders were missing photo identification. The director corrected this at the inspection by obtaining copies of all four employees photo identification and placing them in the employee folders.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

This standard was found to be deficient as the operation lacked infant feeding forms for five children in care.
