746.305(a)(1) - Report Unsafe Facility Occurrence

This standard was evaluated as part of the investigation and was found to be deficient. The operation did not make a self-report to Child Care Regulation when water from a leaky roof entered a room, when water entered another room through the back door/wall of the room, and when moisture was discovered in a wall of a room.

746.501(a)(20) - Written Operational Policies - Parent Rights

The parent handbook was missing a Parent rights statement that included all the items listed in standard 746.521. This was corrected at inspection when the director updated the parent handbook and emailed the new parent handbook to the parents.

746.1205(a)(5) - Responsibilities of Caregivers - Intervene to ensure each child's safety

This standard was evaluated as part of the investigation and was found to be deficient. Through review of video and interviews with children and staff, it was determined a caregiver was not properly supervising the children in care who were using a large basketball game that had two basketball hoops and folding legs. The caregiver was not close to the basketball game and did not intervene to prevent the children from crowding around and leaning on the game which caused the legs to unfold and the equipment to fall. The top of the game hit a child, which caused minor injuries to the child.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

During the walkthrough of the facility, an infant was observed sleeping in a swing. The caregiver stated the infant had been asleep for ten minutes.

746.2427 - AP Infants Sleep on Backs or Must Have Completed and Signed Sleep Exception Form for the Infant

During the walkthrough of the facility, a caregiver was observed placing an infant on their stomach to sleep in their crib.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

During the investigation, video footage was reviewed which showed that a caregiver grabbed two children by the arms and pulled them up and away from a group after the two children were playing on the ground.
