A van used to transport children did not have the child safety alarm installed.

Two out of eight records reviewed did not have current immunizations on file at inspection.

Several slats in the wooden fence in the playground were broken. There were several exposed nails due to the broken slats and the broken slats exposed areas that had broken items.

A wading pool was observed not in use and empty but in reach of children.

A 10 month old child was observed asleep in a play yard with a boppy pillow. Corrected: The boppy pillow was immediately removed from the play yard.

One out of the eight personnel files evaluated did not have a notarized employment affidavit on file.

Two classrooms did not have a written activity plan/schedule posted in the classroom. This was corrected when the director printed the plan and posted them in the classrooms.

The director nor the assigned driver had current transportation safety training on file.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

Based off information gathered, an infant has been left in a non-restrictive device for more than 15 minutes, multiple times.

746.2207(a) - Screen Time Activities - Not Used for Under Two

Children under 2 years old were observed watching a movie. Children eating breakfast were also observed having screen time.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

The oldest child in a group of children was more than 18 months older than the youngest child in the group. The youngest child was 4 months and the oldest was 4 years old.

746.2805(10) - Prohibited Punishments - Using Inappropriate Long Period of Silence or Inactivity Including Requiring Child to Remain in a Restrictive Device

Based off evidence gathered, young children have been expected to sit in chairs and/or remain inactive in the mornings for an inappropriately long period of time, sometimes up to 3 hours.

Based off evidence gathered, a caregiver has yelled at, grabbed, and hit children as a form of discipline.
