746.3407 - Maintenance of Building, Grounds and Equipment

The restroom in the school age class had paper towels and toilet paper on the sink and floor. There was also a plunger laying on the ground sideways. There was a detach peel and stick baseboard molding by the exit door in the school age classroom. The preschool classroom had several staples loose on the floor. The infant classroom had a staple on the carpet by the sleeping equipment. The half-moon table with bucket seats had the safety straps dirty with a brown color. Note. this was corrected at inspection when the bucket seats were removed from the table. The restroom and classroom floors were cleaned. The baseboard taped.

746.4805(a) - Measurement of Use Zone for Slides - in Front of Access and on Sides

The use zones on both sliding equiipment did not have the required six feet distance from each other or other play equipment in the area.

746.4913 - Required Documentation for Unitary Surfacing Materials

The test chart for the turf on the playground was not available for review.

There were several sleeping mats torn in the school-age classroom. Note, this was corrected when the sleeping mats were removed from area.

A caregiver was using their cellphone for personal use while supervising children on the playground.

Two files reviewed lacked immunization record.

A medical authorization form was not signed by parent.

745.635(2) - AP Background check determination - If ineligible then you must immediately remove subject from operation and confirm to CBCU receipt of notice letter

A staff was present at the operation and access to children without an eligible background check.
